Embrace Life Awards Essay Contest

OFFICIAL RULES

  1. NO PURCHASE NECESSARY TO ENTER OR WIN.
  2. ELIGIBILITY: This is an essay writing contest (the "Contest"), open only to individuals who are legal residents of the United States and the provinces of Ontario, New Brunswick and Alberta, Canada, (Florida residents are not eligible to enter this essay contest, but may enter the Florida-only essay contest), who are at least 13 years of age at the time of entry. Employees of State Farm Life Insurance Company (the "Sponsor"), its affiliates, subsidiaries, including its independent contractor insurance and financial services agents, vendors associated with this promotion, the immediate families and members of the same household of these entities, and previous honorees of Embrace Life Awards are not eligible to win.
  3. TO ENTER: Contest begins at 12:01 AM Eastern Time (ET) on January 5, 2009 and ends at 11:59 PM ET on March 2, 2009. Essay contest submitters ("Participants") are invited to nominate a special man or woman ("Nominee") who has persevered and inspired others after the death of a spouse, parent (for purposes of this Contest, "Parent" is defined as parent, step-parent or grandparent), or legal guardian. Nominees must be at least 18 years of age at the time of entry.

    Participants must indicate the state/province where the nominee resides. The nominee's state/province of residence determines which Zone essay contest the essay is entered into. There are 12 Zone essay contests and one (1) winner will be selected from each of the following Zones:

    California
    Canada (Alberta, New Brunswick, Ontario)
    Central (Arkansas, Kansas, Missouri, Oklahoma, Louisiana)
    Great Lakes (Illinois, Indiana, Michigan)
    Great Western (Arizona, Colorado, New Mexico, Nevada, Utah, Wyoming)
    Heartland (Iowa, Minnesota, North Dakota, Nebraska, South Dakota, Wisconsin)
    Mid-America (Kentucky, Ohio, Tennessee)
    Mid-Atlantic (District of Columbia, Delaware, Maryland, North Carolina, Virginia, West Virginia)
    Northeast (Connecticut, Massachusetts, Maine, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, Vermont)
    Pacific Northwest (Alaska, Hawaii, Idaho, Montana, Oregon, Washington)
    Southern (Alabama, Georgia, Mississippi, South Carolina)
    Texas

    Participants are asked to write an essay (of no more than 1,000 words) and include the following weighted criteria: 1.) A brief history of the Nominee and the circumstances leading up to the loss of his/her spouse, parent, or legal guardian (10%). 2.) Information on how the Nominee moved beyond his/her grief to embrace life and inspire others (50%). 3.) How the experience changed him/her (20%). 4.) What, if anything, you think the Nominee would do differently, and what advice, both personally and financially, he/she would offer others facing a similar situation (20%).

    In the event of a tie, the submissions will be reevaluated and ranked on a point scoring system of 1 to 10, (with 10 being the best score), based entirely on criteria #2.

    All entries will be reviewed for eligibility by The US Sweepstakes & Fulfillment Company, the independent contest administrator. Based on the above mentioned criteria, entries will be evaluated by a panel of judges in order to identify the final one (1) Nominee per zone who will be named a winning "Embrace Life Awards Honoree."

    There are two ways to enter: 1.) Visit the State Farm Embrace Life Awards Web site at www.SFEmbraceLifeAwards.com and follow the on-line directions to enter. 2.) Visit the State Farm Embrace Life Awards Web site at www.SFEmbraceLifeAwards.com and print out a hard copy of the application form, which you may then send by mail to the appropriate address below. Along with your essay, be sure to include (for you and your Nominee) complete names and complete addresses, day/evening phone numbers, date of birth (month, day, and year) and a valid e-mail address (if you have one and would like to receive communications by email).

    U.S. residents should mail their entries to: State Farm Embrace Life Awards Essay Contest, 625 Panorama Trail (Bldg. 2 - Suite 100), Rochester, NY 14625. Canadian residents should mail to: State Farm Embrace Life Awards Essay Contest, 17-7000 McLeod Road, Unit 135, Niagara Falls, ON L2G 7K3. All entries must be postmarked by March 2, 2009 and received by March 9, 2009.

    The Sponsor and its agencies are not responsible for lost, late, damaged, misdirected, illegible, postage-due or incomplete mailed submissions, or for online entries that are not received due to technical/transmission difficulties beyond their reasonable control, as stated in "Limits of Liability" paragraph, below. All essays submitted become the exclusive property of the Sponsor and will not be returned.

  4. PRIZES: From all eligible entries received, twelve (12) Embrace Life Awards Honorees (one from each of the above listed Zones) will be selected. Embrace Life Awards Honorees are selected by a panel of qualified judges, under the supervision of the Sponsor, on or about March 31, 2009. One (1) prize of $10,000 (USD) cash will be awarded to the Embrace Life Awards Honoree of each Zone.
  5. CONDITIONS OF PARTICIPATION: By participating, all Participants and Nominees agree to be bound by these Official Rules and the decisions of the Sponsor, which shall be final and binding on all matters relating to this Contest. Submitted essay must be original, based on true events and circumstances, and written expressly for this promotion. Incomplete, "enhanced" or fictional, and/or illegible entries will be disqualified. Entries shall not contain profanity, graphic or sexually explicit material. Any entry deemed inappropriate at the discretion of the judges and Sponsor will be disqualified. By entering this contest, Participant agrees that the Sponsor or his/her assigned agent(s) will have the right to use, adapt, modify, revise, reproduce and/or publish the essay, in any manner or form throughout the world, in perpetuity without compensation.

    Embrace Life Awards Honorees will be required to execute and return (the "Required Documents") an Affidavit of Eligibility, Liability/Publicity Release and a copy of their valid driver's license (front and back), within ten (10) days after issuance of written notification. A background check will be performed. If in Sponsor's reasonable discretion the background check results are unacceptable, such Nominee will be disqualified from the Contest. The Required Documents grant the Sponsor the right to confirm the validity of Participant's entry and to use Participant's and Nominee's name and likeness in all publicity and marketing activities associated with this Contest, including obtaining and posting photos, audio, video and b-roll of Nominee online and the right to use, reprint, adapt, revise, modify, reproduce and/or publish the essay, photos, audio, video and b-roll in any form of media, without further compensation. Noncompliance with any of the Required Document requirements outlined above may result in disqualification and selection of an alternate winner.

    The Participant who submits a winning entry does not have independent rights to the prize for any circumstances, including if Embrace Life Awards Honoree is disqualified for non-compliance, or any other reason. Embrace Life Awards Honorees must be at least 18 years of age at the time of entry. Any attempt by any individual to deliberately undermine the legitimate operation of this Contest is a violation of criminal and/or civil laws. Should such an attempt be made, Sponsor reserves the right to disqualify said individual and seek damages and other remedies to the fullest extent permitted by law. Prizes are not transferable and no substitutions are allowed, except at the sole discretion of the Sponsor. All Participants, Embrace Life Awards Honorees further agree that Sponsor and each of its respective affiliates, subsidiaries, employees and advertising and promotion agencies shall have no liability and will be held harmless for any liability, loss, injury or damage due to acceptance, use or misuse of the prize offered, and participation in any prize-related activities or any travel related thereto, or any other aspect of participation in this Contest. All federal, state/provincial and local laws apply. All expenses not specified herein, and all taxes on prizes, are the sole responsibility of each Embrace Life Awards Honoree.

  6. LIMITATIONS OF LIABILITY: Sponsor and Contest Administrator are not responsible for any incorrect or inaccurate information, whether caused by Web site users or by any of the equipment or programming associated with, or utilized in, this Contest, or by any technical or human error which may occur in the processing and/or transmissions in the Contest. Sponsor and Contest Administrator assume no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or authorized access to, or alteration of, entries. Sponsor and Contest Administrator are not responsible for any problems or technical malfunction of any telephone network or lines, computer on-line systems, servers or providers, computer equipment, software, failure of e-mail due to technical problems or traffic congestion on the Internet or at any website or combination thereof, including injury or damage to participants or to any other person's computer related to, or resulting from, participating or downloading materials in this contest. If, for any reason, the Contest is not capable of running as planned, including, without limitation, infection by computer virus, bugs, worms, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor or Contest Administrator, which corrupt or affect the administration, security, fairness, integrity or proper conduct of this contest, Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the Internet portion of the Contest, and select winners from all Internet entries submitted prior to the problem and from mail entries.
